In the month of April, a two-year-old boy from Iraq was admitted to Medanta Hospital for treatment of a rare sarcoma known as Myofibroblastoma. He arrived in a critical condition with a 10 cm × 6 cm tumor affecting his cheek, mouth and throat, leaving him unable to eat and severely malnourished.

A multidisciplinary team of experts, including India’s leading oncologists, reconstructive surgeons, and neurosurgeons, was quickly assembled under the Medanta Tumor board to handle the complex case. The team comprised Dr. Deepak Sarin, Dr. S.P. Yadav, Dr. Shandip Sinha, Dr. Rajiv Uttam, Dr. Rakesh Kumar Khazanchi, and Dr. Neeru Bansal.

A CT scan revealed that the tumor extended to the skull base but with no significant attachment to the mouth or throat, making surgical removal their best bet. However, for the resection to take place, the 2-year-old had to be in a stable condition with improved nutrition and a reduced tumor size.

Dr. S.P. Yadav, Director of Paediatric Oncology at Medanta Hospital, shared details about the per-surgical care, ‘Surgery wouldn’t have been possible without medications. Medications were used to shrink the tumor and a gastrostomy was performed to improve the child’s nutrition intake. By the time of surgery, the tumor had shrunk by approximately 70%, making the child stable enough for the operation.

A 14-hour surgery was then undertaken, during which the medical team removed the tumor along with the buccal mucosa, part of the upper jaw, and the entire lower left jaw. The tumor was successfully resected, but this extensive procedure also required a major facial reconstruction. Medanta’s expert reconstruction team was able to restore approximately 60–70% of the child’s facial structure.

Post-operative was equally important to the success of the surgery. Dr. Shandip Sinha and Dr. Rajiv Uttam closely managed his nutrition, infection control, and respiratory support. Within two weeks, the child transitioned to a soft food diet and continues to make steady progress under the follow-up care of the Medanta Tumor Board.
